- Their program (After school care, Toddler, Preschool or Prek)
- Full time or Part time (if part time, list the days needed)
- School or n/a
- Grade or n/a
Forms are not needed by Tuesday, Aug. 3 but in case you want to get a jump start, Click here for forms.
All forms must be filled out prior to your child’s care date. We can not pick up your child from school until forms are completed and returned to the clubhouse.
